Consent to Personal Data Processing

I, the personal data subject, by filling in and submitting a form on the site https://domresnic.ru, freely, of my own will and in my own interest, give consent to Sole proprietor Ekaterina Pavlovna Shpak (hereinafter — the Operator), whose details are given below, to process my personal data on the terms set out below.

1. The list of personal data for the processing of which consent is given:

• surname, first name (and patronymic, if any);
• phone number;
• email address;
• the information I provide in the form fields (desired service, date and time of visit, comment).

2. Purposes of processing:

• receiving and processing a request/booking for services;
• contacting me to confirm the booking, remind me of the visit and respond to enquiries;
• concluding and performing the service agreement to which I am a party/beneficiary;
• keeping a history of my visits for the provision of services.

3. The list of actions with personal data and the methods of processing.

Consent is given for the following actions: collection, recording, systematisation, accumulation, storage, clarification (updating, modification), extraction, use, transfer (provision, access) to a person processing the data on the Operator's instructions, blocking, deletion, destruction. Processing is carried out with and without the use of automation tools (mixed), without cross-border transfer.

4. Processing on instruction.

I am notified and agree that, to process requests, the Operator may entrust the processing of my data to a party providing online-booking services (the YCLIENTS service), while the Operator remains responsible to me.

5. Validity period and withdrawal procedure.

The consent is valid from the moment it is given until the purposes of processing are achieved or until it is withdrawn. I have the right to withdraw consent at any time by sending a statement to the email info@domresnic.ru or to the Operator's address. After withdrawal, the Operator stops processing and destroys (anonymises) the data within the periods established by law, except for data whose storage is required by the legislation of the Russian Federation.

6. Confirmation.

I confirm that I have read the Personal Data Processing Policy, act voluntarily, that the data provided is accurate and relates to me, and that my rights under Federal Law No. 152-FZ have been explained to me.
